Job Description:
A Support Worker in community care plays a vital role in
assisting individuals who require extra support due to age,
disability, chronic illness, or mental health conditions. Their
primary focus is to enable clients to maintain independence,
dignity, and a high quality of life while remaining in their own
homes or local communities.
Requirements:

Valid NDIS Worker Screening Check.

Current Police Check.

Working With Children Check.
-
First Aid and CPR Certificate.

Certificate III or IV in Individual Support / Disability /
Community Services.

Experience in community care, NDIS, or aged care.

Strong communication and interpersonal skills.

Compassionate, patient, and respectful attitude.

Ability to work independently and as part of a team.

Good time management and reliability.

Valid driver’s licence and reliable vehicle.

Willingness to travel within the community.

Flexible availability, including weekends and evenings.

Up-to-date vaccinations as per organisational requirements.
Responsibilities:

Assist clients with personal care (showering, dressing,
grooming, toileting).

Support clients with daily living activities and routines.
-
Provide domestic assistance such as cleaning, laundry, and
meal preparation.

Support community access including shopping, appointments, and
social activities.

Encourage and promote client independence and wellbeing.
-
Assist with mobility and transfers safely.
-
Provide emotional support and companionship.
-
Follow individual care plans and behaviour support plans.
-
Observe, monitor, and report changes in client health or
behaviour.

Complete daily notes, progress reports, and incident reports.
-
Maintain client privacy, dignity, and confidentiality.
-
Follow WHS, infection control, and organisational policies.
-
Communicate effectively with coordinators, families, and
healthcare professionals.
